Only hermanas narrows it to sisters, and Spanish speakers add the detail when it matters — dos hermanos, un chico y una chica.
The word comes from Latin germanus, of the same parents — the root behind English cousin-german and the adjective germane, and nothing whatever to do with Germany. Two neighbours are worth keeping apart: media hermana is a half-sister, sharing one parent, while hermanastra is a stepsister, sharing none. Capitalised, Hermana is how a nun is addressed.
